Tri-State Community Health Center (Tri-State) was incorporated as a private non-profit entity in 1987. Tri-State was established by the community to help assure that comprehensive primary health care services would be available to the area’s under-served rural population with a high concentration of low-income residents. Tri-State operates four community health center sites that offer community-based primary care, preventive health services and OB/GYN services to low income and uninsured residents. The sites are located in Allegany and Washington counties in Maryland and Fulton County, Pennsylvania and serves residents in each of these counties as well as the surrounding areas. The primary target population for Tri-State Community Health Center is low-income, uninsured and underinsured individuals and families in Allegany County and Washington County, Maryland; Fulton County, Pennsylvania; and Mineral and Morgan County, West Virginia. All four sites are located in extremely rural communities with limited employment opportunities and medical services. Due to the limitations, these communities are economically challenged and have high rates of uninsured, unemployment and poverty. Tri-State locations, partnerships, outreach and comprehensive services allow the organization to address the unique needs of the rural population.

The organization has grown steadily in health center users and medical visits due to the expansions and the increase in need. In 2005-2006, Tri-State provided services to 16,166 community residents facilitating 52,670 encounters. Over 45 percent of the users were below 200 percent of poverty and 33 percent were uninsured, underinsured and/or receiving medical assistance. These services are currently provided through the employment of ten physicians, four nurse practitioners, one physician assistant and a wide range of clinical and administrative support staff, creating over 120 jobs in the communities served by Tri-State.

The health centers offers, directly and through collaborative arrangements, a wide range of general primary care and health maintenance services, CLIA-waived diagnostic laboratory services and other diagnostic tests and screenings, family planning, urgent medical care, after-hours coverage, immunizations, HIV testing, gynecological care, mental health treatment and counseling, developmental screening, vision screening, hearing screening, oral health screening, health education, transportation, translation, and case management services. Services provided off-site through referral arrangements include obstetrical care, mental health and substance abuse treatment and counseling, developmental screening, oral health services, crisis intervention, and hospital care.

Combined and at full capacity, all sites of Tri-State will serve over 26,000 users with over 76,000 encounters per year by the end of the project period in FY 2010.
